Is it possible to initialize and output content to a display monitor that the Windows desktop hasn't been extended to? In Windows 7 terms such a display is referred to as "disconnected" although it is physically connected to a video adapter output.

It seems like the Desktop Window Manager stands in between the DXGI SwapChain and the device hardware and copies surfaces across video adapters boundaries which greatly impairs the SwapChain's both frame rate ability and stability. We observe this behavior in multi-adapter configurations based on both ATI and NVidia video hardware.
